The Hospice Care Team

The hospice care team is a special group of healthcare professionals who focus on patients with life-limiting illnesses and their families.

Generally, the team includes a physician, nurse, hospice aide, social worker, volunteer, chaplain, and bereavement specialist. 

For after-hours needs, VITAS provides Telecare, which is 24-hour telephone access to hospice clinicians who can answer questions, support caregivers over the phone or dispatch a team member to the bedside, if needed.
